Which German hospitals answer a doctor's cold email
Cold emails to German hospitals work best on short-staffed departments: anesthesia, internal medicine and emergency, psychiatry. Radiology and dermatology rarely reply.

A cold email to a German hospital with no open job is called an Initiativbewerbung. Most of them go unanswered. The ones that get a reply almost always land in one kind of department: the ones short on doctors.
Which departments usually reply
Anesthesia, internal medicine and emergency departments, and psychiatry wards reply the most. These departments run night and weekend duty (Bereitschaftsdienst) with thin rosters, and a missing doctor means someone else works a double shift.
A chief doctor (Chefarzt) running an anesthesia department with 2 open contracts does not wait for an advert to close. He reads the email that morning.
Which departments usually do not
Radiology, dermatology, and ENT rarely answer a cold email. These specialties get long queues of German applicants for every open contract, so there is no shortage pushing anyone to read unsolicited mail.

Doctors are listed as a shortage occupation under Germany's EU Blue Card rules. This tells you where the actual gap is. It is wards that run around the clock, not outpatient specialty clinics with fixed hours.
Why this happens
A department only replies when a real gap costs someone sleep. Internal medicine and emergency wards run 24 hours.
Anesthesia covers every surgery and every emergency call. Psychiatry wards carry duty rosters too.
Radiology and dermatology mostly work daytime hours with no night duty gap to fill. A missing doctor there is inconvenient, not a crisis.
What to write
- Name the department, not just the hospital: "Klinik für Anästhesiologie", not just the hospital name.
- Say your specialty and your current paper status: licence (Approbation), temporary work permit (Berufserlaubnis), or the exam date you already booked.
- Say when you can start, with a real month, not "soon".
- Attach your CV and your licence or exam certificate as one file, not five.
What to do
- Make a list of hospitals within 1 to 2 hours of the city you want, and sort by department, not by hospital name.
- Send to anesthesia, internal medicine or emergency, and psychiatry first.
- Address the chief doctor (Chefarzt) or deputy by name, found on the hospital website.
- Follow up once after 2 weeks if no reply, by the same email thread.
Stand: 2026-11-01. This is a guide, not legal advice. Every German state has its own rules, and rules change. Check with your state's medical chamber (Landesärztekammer) before you act.
