Germany can extend a temporary doctor permit
Germany can give a doctor an unlimited temporary permit in rare medical cases. It is still not a full doctor licence and can stay limited or be withdrawn.

An unlimited temporary permit in Germany is rare. It does not give you a full doctor licence.
The state licensing office can still limit your hospital, specialty, or tasks. Read every line of the permit letter.
This rule matters when your temporary work permit is ending. A hospital may tell you that an unlimited permit solves everything.
It does not. You still work under temporary permission, not a full licence.
What is the normal rule?
The normal temporary work permit is called a professional permit (Berufserlaubnis). It lets a doctor work before receiving a full doctor licence.
The full doctor licence is called Approbation. It allows you to practise medicine across Germany.
The licensing office in your German state gives the permit. The office can withdraw it.
The letter can name one hospital, one department, or one supervising senior doctor. Those limits apply to your daily work.
The Federal Medical Practitioners Act sets a normal total limit of 2 years. This includes work under temporary permits anywhere in Germany.

When can it have no end date?
The law allows an unlimited permit only in a medical exceptional case. The licensing office must see a patient-care reason for you to stay.
This is a narrow exception. A difficult licence process does not create a medical exceptional case.
A failed exam does not create one either. A hospital contract alone is also not enough.
The strongest file names a real local need. A rural hospital may show missing doctors in anaesthesia, emergency care, or psychiatry.
The hospital must explain why it needs you in that department. It should name your job, specialty, and place of work.
The state office makes the decision, not the hospital chief. A chief can write a support letter for your file.
The licensing office decides whether the legal exception applies. The chief cannot grant the permit.
Why is it still temporary?
An unlimited permit means the letter has no end date. It does not remove the other limits in that letter.
Your letter may say that you work only at Klinikum X. It may require supervision by Dr Y.
It may limit you to internal medicine. It may also exclude private practice.
A full doctor licence is different. Approbation lets you practise throughout Germany without a hospital-specific work limit.
The licensing office can still act if legal requirements later fail. That rule also applies to doctors with a full licence.

Do not stop your licence application
Keep working on your full licence application. An unlimited permit keeps a needed doctor in one department.
It does not replace the language exam, knowledge exam, or licence decision. You still need the full licence for unrestricted work.
This matters when you change hospitals. Your new hospital may not fit the limits in your old permit letter.
Read the letter before signing a new contract. A new department may need a new decision from the licensing office.
What to do now
- Read your current permit letter. Mark every limit on hospital, specialty, supervision, and end date.
- Ask the licensing office in writing: “Can I apply for an unlimited Berufserlaubnis under the medical exceptional-case rule?”
- Ask your hospital for a signed letter naming the specialty, local staffing need, and your exact job.
- Continue your full licence application. Keep copies of every exam result and office letter.
