Berufserlaubnis Before Approbation: Paid Doctor Work in Germany

A Berufserlaubnis can lead to paid hospital work before Approbation, but only with a job, supervision, and the limits set by your state licensing office.

A Berufserlaubnis gets you paid doctor work in Germany only when a hospital hires you for supervised work under the limits written on the permit. It is a temporary permission to practise medicine before full Approbation, Germany's unrestricted medical licence. It does not turn an unfinished licence application into a job offer.

This matters when you are waiting for the Kenntnisprüfung, the medical knowledge exam for doctors whose degree is not judged equal to German training. A ward in Bremen, a rehabilitation clinic in Bavaria, or a rural internal medicine department may hire you while your Approbation file is still open. The state licensing office decides whether you receive the permit, and the hospital decides whether it wants to employ you.

Can you earn a salary with a Berufserlaubnis?

Yes, you can earn a salary as a junior doctor when the Berufserlaubnis allows employment in a hospital or practice. You sign an employment contract, join the rota, examine patients, write discharge letters, take blood, and work on the ward. Your exact duties depend on the conditions printed on your permit and the doctor supervising you.

The common arrangement is dependent employment under medical supervision. You work in one department with a named senior doctor or chief responsible for supervision. A Berufserlaubnis does not normally allow you to open your own practice, replace a fully licensed doctor without oversight, or move freely between hospitals.

Section 10 of the Bundesärzteordnung, the federal medical licensing law, gives states the legal basis to issue this temporary permission. The law limits it to supervised or dependent work and sets a general maximum period. Your state authority writes the practical restrictions into the document you receive.

Why the hospital comes before the permit

In many states, the licensing office wants to see a concrete job offer or employment contract. The document names the hospital, department, city, planned start date, and supervising doctor. A generic email saying that you are interested in surgery is not enough for this file.

This creates the difficult part: hospitals prefer doctors who can start safely and legally, while the office may want a hospital contract before issuing the permit. You solve that circle by applying honestly. Write in your cover letter that your Approbation application is open and that you are applying for a position under a Berufserlaubnis.

A chief in a small hospital may accept this because the department needs a junior doctor for ward rounds, admissions, and night duty. A university clinic with 30 applicants for one post usually chooses the doctor who already holds Approbation. The permit is strongest in departments with open rota gaps, not in highly competitive city jobs.

Your permit can be tied to one place

Yes, a Berufserlaubnis is commonly tied to the Bundesland, employer, department, or specialty named by the licensing office. Read every line before you accept the contract. If the document says internal medicine at a named hospital in North Rhine-Westphalia, you cannot use it for an orthopaedics post in Hamburg.

Berlin's licensing service explains that a temporary permission is issued for work under supervision and can carry restrictions. Baden-Württemberg also handles temporary professional permission through its recognition authorities, with the employer and intended work forming part of the application. The exact form differs by state, but the permit is never a blank cheque for Germany.

Changing hospitals is where doctors lose time. A new contract can require a changed permit or a fresh decision from the licensing office. Do not resign from your first hospital because another department sends a friendly email. First ask the office whether your existing Berufserlaubnis covers the new employer.

What supervision looks like on a real ward

Supervision is not a signature at the end of the month. On an internal medicine ward, a senior doctor reviews your admission plan, medication changes, discharge letters, and difficult patient conversations. During night and weekend duty, the hospital must organise senior backup that matches your permit conditions.

The level of oversight differs between a first job in Germany and a doctor who has already worked for months in the same department. But the hospital remains responsible for how you work. If a chief tells you that you will cover an emergency department alone from your first week, show the job description to the licensing office before signing.

What a Berufserlaubnis cannot fix

A Berufserlaubnis cannot fix missing language papers, an incomplete identity document, or a failed requirement from the licensing authority. It also does not remove the Approbation process. You still submit certificates, translations, a certificate of good standing, health papers, and the documents required for the Fachsprachprüfung or Kenntnisprüfung.

It also does not guarantee that every month becomes specialist training. If you want Facharzt training, ask the state medical chamber whether the department and your work under this permit can be entered in your Logbuch, the training record. The chamber decides training credit, while the licensing office decides the Berufserlaubnis.

The best use of a Berufserlaubnis is simple: it puts you inside a German hospital while you finish Approbation. You earn a salary, learn the electronic patient record, hear German during ward rounds, and collect a German employment reference. It remains temporary permission with written limits, not full freedom to practise.

What to do before you accept a job

  • Ask the licensing office in writing: “Can my Berufserlaubnis cover this hospital, this department, and night duty under the attached contract?”
  • Ask the chief for the name and title of the doctor who supervises you on the ward and during night and weekend duty.
  • Send the hospital your current Approbation application receipt, language certificate, CV, and the list of documents still requested by the licensing office.
  • Ask the state medical chamber whether work in this named department can be entered in your Facharzt Logbuch while you hold a Berufserlaubnis.
