A Fee Rule Could Cut Lab Doctor Pay in Germany

A draft change to Germany's doctors' fee schedule (GOÄ) could cut pay for lab doctors. Check whether your target lab is hospital-paid or fee-based before you apply.

A Fee Rule Could Cut Lab Doctor Pay: A draft change to Germany's doctors' fee schedule (GOÄ) hits private labs, not hospital lab jobs.

A proposed change to Germany's doctors' fee schedule, the GOÄ, could cut income for lab doctors (Laborärzte). It will not hit every lab job the same way. Before you apply to a lab medicine (Labormedizin) post, find out how that lab actually gets paid.

What is the GOÄ, and what is changing?

The GOÄ is the price list German doctors use to bill private patients and many lab tests. Lab doctors depend on it for a large share of their income, more than most specialties.

A draft revision is being discussed in Berlin, and lab doctor associations are warning it could lower what a lab earns per test. This is a draft. No date is fixed, and nothing is law yet.

Why lab doctors are worried

Routine blood tests, hormone panels and most lab orders get billed per test. If the fee per test drops, a lab's income drops too, even with the same number of tests.

Lab doctor groups say a cut this size could squeeze budgets at the labs that also train junior doctors.

Does this change your paycheck?

It depends on who signs your contract, not on your specialty.

A hospital lab department, inside a public hospital, pays you from the normal doctors' collective pay table (TV-Ärzte). Your salary does not move with the GOÄ.

A private lab or a medical treatment centre (MVZ) runs on the fees it bills. If those fees drop, it has less money for new hires, bonuses or overtime pay.

A split screen compares a hospital lab department, paid from the normal pay table, against a private lab, paid from GOÄ billing.

How to check before you apply

Ask the lab two direct questions before you sign anything.

  1. Is this job inside a hospital or university lab, paid from the normal pay table?
  2. Or is it a private lab or MVZ, paid from its own billing?
  3. Has the lab mentioned a hiring freeze or cut bonuses this year?
A checklist of three questions to ask a lab before applying there.

A hospital lab job keeps you out of this fight over fees. A private lab job can still be a good offer, just ask about its finances first.

What to do now

  • Ask HR in writing: is this position paid from the hospital pay table or from lab billing?
  • Search the hospital's name plus 'GOÄ' or 'Labor' news from this year before you apply.
  • Compare at least one hospital-lab offer against one private-lab offer before deciding.
