Do not pay for an electronic doctor card before your German hospital confirms the costs in writing. The card is personal. The hospital may need it for your daily work, but that does not make a verbal promise about repayment safe.

An electronic health professional card (elektronischer Heilberufsausweis, eHBA) lets doctors make a qualified electronic signature. Hospitals use this for tasks such as signing electronic prescriptions or medical letters in connected systems.

When can a hospital require it?

A hospital can require an eHBA when your assigned work needs your personal electronic signature. This is common when you sign electronic prescriptions, digital discharge letters, or other documents in the hospital system.

The card belongs to you, not the ward. You order it through the responsible state medical chamber or its approved card provider. Your full doctor licence or temporary doctor permit is checked during the application.

A chief saying, “You need this for the ward,” answers only the work question. It does not answer who pays the card provider, the identification process, replacement cards, or later renewal.

Why five years can trap you

The certificates on an eHBA have a limited validity period. A card order can therefore create a long contract with regular charges. Read the provider’s order form before you enter your bank details.

A hospital may offer reimbursement after you submit each invoice. Another hospital may pay the provider directly. The risky version is a repayment clause that asks for money back if you change hospitals before an agreed period ends.

Junior doctors change hospitals for specialty training, family reasons, and better jobs. A five-year repayment clause can follow you long after one rotation ends. Do not accept it just because the ward needs a card this month.

Get these terms in writing

Ask Human Resources, not only the senior doctor on your ward. Human Resources can add payment terms to your contract, offer letter, or a separate signed letter.

  • Who pays the card provider, including identity checks and delivery.
  • Whether the hospital pays first or refunds your invoice later.
  • The maximum amount the hospital will refund each year.
  • Whether replacement, renewal, and revocation costs are included.
  • Whether you repay anything after leaving, and the exact calculation.

A repayment clause in an employment contract is not automatically valid. German civil law checks standard contract terms for unfair disadvantage. Let a union adviser or employment lawyer read a long repayment clause before signing it.

What to do before ordering

  1. Email Human Resources: “Will the hospital pay my eHBA costs directly or reimburse every invoice?”
  2. Ask for the answer in your employment contract or a signed letter.
  3. Request the full repayment clause before ordering the card.
  4. Keep the provider order form, invoices, and the hospital’s written promise in one folder.
