German junior doctors: fix insurance before day one
Before your first junior doctor job in Germany, tell your insurer about your hospital duties and read every exclusion in writing.

Before your first junior doctor job in Germany starts, change your work disability insurance from student or nursing work to your real hospital duties. Your new contract brings night shifts, blood draws, emergency calls and ward rounds.
Do this before day one. A policy paper with the wrong job description can cause a hard argument years later.
Why your first contract matters
Work disability insurance (Berufsunfähigkeitsversicherung, BU) pays if illness or injury stops you doing your job. The policy must describe the job you actually do.
A student spends time in lectures and exams. A nurse has different tasks from a junior doctor. Your first hospital contract changes the work written on your insurance file.
Write down your daily tasks before calling the insurer. Include admissions, discharge letters, blood draws, ward rounds, emergency room work and night and weekend duty (Bereitschaftsdienst).
Read these 4 lines first
Open the policy conditions and find the section about your occupation. Do not rely on a sales call or a short email summary.
- Your occupation: Does it say junior doctor, hospital doctor, or your old student job?
- Work tasks: Does the insurer have your current duties in writing?
- Night duty: Is night, weekend and emergency work named or excluded?
- Restrictions: Does the policy exclude mental health, back pain, surgery, or a past illness?

Do not hide a health answer
Answer every written health question fully and truthfully. Keep copies of the form, your answers and every doctor letter you send.
German insurance law says you must disclose known risk details when the insurer asks for them in text form. The insurer must ask the question clearly.
A knee MRI, antidepressant prescription, therapy visit or sick note can matter if the form asks about it. Send the actual report when the insurer requests it.
Ask for written answers
Call the insurer if you need to. Then send an email the same day. Ask them to confirm the new occupation and every change in writing.
Use one clear sentence: “From my first workday, I am employed as a junior doctor in a German hospital. Please confirm my insured occupation and all exclusions.”
Check the policy definition
Read how the policy defines being unable to work. Look for the percentage of lost work ability, the waiting period and the insurer’s right to move you into another job.
The policy wording decides the payment test. A friendly answer from an agent does not replace the policy document.
What to do before day one
- Email the insurer your signed junior doctor contract and first work date.
- Ask for the insured occupation and all exclusions in writing.
- Save the policy, health form, email replies and hospital contract in one folder.
- Read the section about night duty, mental health and changing jobs before you start work.
Your first doctor contract is the moment to fix this paper. Do it before the first night shift, not after a health problem.
Stand: 2026-10-15. This is a guide, not legal advice. Every German state has its own rules, and rules change. Check with your state's [medical chamber (Landesärztekammer)](https://www.bundesaerztekammer.de/aerztekammern) before you act.
