Why German specialist jobs feel closed
Dermatology, radiology, and ophthalmology are difficult for different reasons. Target the real barrier instead of treating all three as impossible.

Dermatology, radiology, and ophthalmology are hard for foreign doctors in Germany for different reasons. Do not treat all 3 specialties as one closed door.
A dermatology chief looks closely at your German patient communication. A radiology department looks for hospital experience with reports and imaging systems. Ophthalmology has fewer junior doctor posts to begin with.
Three fields, three barriers
The same CV will lose in all 3 fields. It loses for a different reason each time.
- Dermatology: spoken German and patient consultations.
- Radiology: German hospital work and precise written reports.
- Ophthalmology: few entry jobs and strong competition.
- University clinics: many applicants with German research or electives.
Dermatology needs visible German experience
Dermatology is patient-facing from the first clinic day. You explain rashes, biopsies, creams, side effects, and skin cancer results in short consultations.
A chief sees a foreign degree and asks one practical question. Can this doctor speak clearly with a worried patient in a busy outpatient clinic?
Applicants with German ward work, a dermatology observership, or a German reference letter usually look stronger. A generic application from abroad usually looks distant.
Your first move is simple. Put every skin-related task on page 1 of your CV, including biopsies, wound checks, allergy work, and outpatient consultations.
Radiology needs hospital routine
Radiology has less bedside conversation than dermatology. But every CT report, phone call to surgery, and urgent night report needs exact German.
University radiology departments also receive applications from doctors with German research, electives, and local contacts. A foreign doctor without German hospital work starts behind them.
Small hospitals give you a more realistic first target. Their radiology teams still need doctors who can write reports, speak to emergency doctors, and cover night work.

Ophthalmology has fewer first jobs
Ophthalmology is difficult because there are fewer junior doctor posts. Many treatments happen in outpatient clinics, so hospitals have smaller training teams than large internal medicine departments.
The applicant who wins is not always the doctor with the longest CV. It is often the doctor who has shown real interest through an ophthalmology elective, a research project, or work in an eye clinic.
Do not send 50 identical emails to university eye clinics. Write to hospital departments and eye clinics in smaller cities, then name the exact work you want to learn.
University clinics are a separate race
A university clinic is not the normal first job for most applicants. It competes for doctors who already know the department through research, a German elective, or a student placement.
Apply there if you have that link. Otherwise, apply first to a non-university hospital where the department needs a junior doctor for ward work, admissions, and night duty.
What to do this week
- Make 3 CV versions, one each for dermatology, radiology, and ophthalmology.
- Add 4 concrete tasks from your work that fit the specialty.
- Email one department: “Do you have a junior doctor vacancy or observership in ophthalmology?”
- Apply to smaller hospitals before university clinics.
- Ask each department which German experience they expect before interview.
These specialties are difficult. They are not difficult for one single reason. Target the barrier in front of you, then your application starts to make sense.
Stand: 2026-10-12. This is a guide, not legal advice. Every German state has its own rules, and rules change. Check with your state's [medical chamber (Landesärztekammer)](https://www.bundesaerztekammer.de/aerztekammern) before you act.
