Can doctors observe in Germany on a visitor visa?
A short unpaid hospital observership can fit a visitor visa in Germany if you only observe. Patient work needs a permit that allows work.

A visitor visa is suitable for a short unpaid hospital observership only when you watch and do no doctor’s work. The moment you examine patients, write notes, take blood, or join the rota, you need permission to work.
This matters because a hospital invitation letter is not a work permit. The German embassy decides your visa. The local immigration office decides later permits in Germany.
What you may do as an observer
A hospital observership is called a Hospitation in Germany. In a real observership, you stand beside a senior doctor and watch the day.
You can sit in morning handover. You can watch ward rounds, an operation, an ultrasound, or a patient discussion. You can ask questions afterwards.
- Watch ward rounds, operations, and outpatient clinics
- Attend teaching meetings and case discussions
- Read anonymised discharge letters with your supervisor
- Meet the chief and learn how the department works
You do not replace a junior doctor. You do not treat a patient. You do not receive payment from the hospital.

Where the line is
The line is simple: watching is observation. Doing a task that helps the hospital run is work, even if nobody pays you.
Taking a blood sample, examining an abdomen, writing a discharge letter, or covering a night shift crosses that line. These tasks involve patients and belong to employed doctors.
Your visitor visa does not give you a temporary work permit (Berufserlaubnis). Without that permit, you cannot work as a doctor in a German hospital.
When another visa is safer
Choose a recognition visa if your hospital stay is part of getting your German doctor licence. This visa is made for recognition steps, language courses, exams, and related training.
The recognition visa can run for up to 24 months. It can be extended by 12 months, up to 3 years in total, as of October 2026.
It also allows a side job of 20 hours each week. It does not itself allow doctor work. For ward work, you still need the temporary work permit from the licensing office.
Get the letter right
Ask the hospital for an invitation letter before your visa appointment. A vague letter saying “clinical training” can create problems at the embassy border desk.
The letter should state that the placement is unpaid. It should say you observe only and perform no patient care, medical procedures, documentation, or night duty.
It should also name the department, supervisor, start date, end date, and hospital address. Take the signed letter with you when you travel.
What to do now
- Ask the hospital for a letter that says “unpaid observation only” and lists no patient tasks.
- Send that letter to the German embassy before your visa appointment if the wording is unclear.
- Choose the recognition visa when your stay includes a language course, exam, or licensing steps.
- Do not start ward work until the licensing office issues your temporary work permit.
