German doctors need more than 20 work hours
The Opportunity Card allows 20 work hours each week, but foreign doctors need a temporary work permit or full licence for clinical doctor work.

The Opportunity Card lets you work up to 20 hours each week in Germany. It does not let you work as a doctor. Clinical doctor work needs a temporary work permit or your full doctor licence.
Book your flight with this difference in mind. A hospital job offer and a residence card are different papers.
The hospital hires you. The state licensing office decides if you may examine patients, prescribe medicines, or work on a ward.
What the 20 hours mean
The Opportunity Card is a job-seeker residence permit. It lasts up to 1 year.
It allows 20 hours of work each week. It also allows trial jobs lasting up to 2 weeks each (as of October 2026).
Use those hours for work outside medicine. Picture a café shift, tutoring, warehouse work, or an office job.
That money can help with rent. You can study German and prepare your licence papers afterwards.
A hospital can offer a non-doctor role. The written job description must name the tasks clearly.
You cannot be the responsible ward doctor with an Opportunity Card alone. A hospital badge does not change that rule.

Why hospital work needs another paper
Clinical doctor work includes taking responsibility for patients, ordering treatment, and writing prescriptions. It also includes night and weekend duty.
Germany protects these tasks through its doctor licence system. Your residence card does not replace that system.
Before full recognition, some states issue a temporary work permit (Berufserlaubnis). This paper lets you work as a doctor under state limits.
The permit lasts at most 2 years in total (as of October 2026). The state licensing office sets the exact limits.
Your employer cannot replace the permit with a contract. A department letter or a chief's promise also does not replace it.
Do not confuse these 4 papers
- Opportunity Card: lets you live in Germany and work up to 20 hours weekly.
- Hospital contract: says the hospital wants to hire you for a named job.
- Temporary work permit: lets you work clinically before your full doctor licence.
- Full doctor licence: lets you work independently as a doctor.
Use the card for the right job
Use the Opportunity Card to arrive, improve German, attend interviews, and earn money outside doctor work. Start your doctor licence application from abroad if possible.
You can apply from abroad in every German state. Each licensing office asks for slightly different papers.
If a hospital offers a junior doctor post, ask which licensing office receives your permit application. Ask the human resources office for the written offer.
Also ask for the planned start date. Keep that letter with your passport and licence papers.
What to do now
- Write down 3 non-doctor jobs you could legally do for 20 hours.
- Send your doctor licence application to the licensing office in your chosen state.
- Ask the hospital: Which office handles my temporary work permit application?
- Keep your hospital contract and licence papers in one folder for your visa appointment.
Stand: 2026-10-14. This is a guide, not legal advice. Every German state has its own rules, and rules change. Check with your state's [medical chamber (Landesärztekammer)](https://www.bundesaerztekammer.de/aerztekammern) before you act.
