    Germany: 3 Internal Medicine Years Before Endocrinology

    Your first 3 Internal Medicine years support endocrinology in Germany, but the state medical chamber decides which rotations and training licence count.

    Your first 3 years in Internal Medicine build the foundation for endocrinology in Germany. They are not wasted years. But you need the correct department, training licence, and Logbuch entries before you spend years on a ward that your state medical chamber will not certify for your endocrinology qualification.

    The important correction is this: there is no single national shortcut called Zusatzweiterbildung Endokrinologie that every doctor follows. Germany has a model training rule from the Federal Medical Association, but each Landesärztekammer, the state medical chamber, writes and applies its own binding training rules. In many states, endocrinology is a Schwerpunkt, an advanced Internal Medicine specialty, called Endokrinologie und Diabetologie.

    Do the first 3 Internal Medicine years count?

    Yes. The first 3 years are the base of the common Internal Medicine training period in the usual endocrinology specialist route. A junior doctor on a general Internal Medicine ward learns admissions, discharge letters, ECG interpretation, insulin adjustments, electrolyte problems, heart failure treatment, night duty, and emergency work. Those are the hospital tasks endocrinology departments expect before they give you complex diabetes and hormone patients.

    The exact months are not interchangeable. Your chamber looks at the approved Weiterbildungsstätte, the hospital or practice approved for training, and the Weiterbildungsermächtigung, the senior doctor's training licence. A department can employ you for 3 years while its licence covers only part of the time required for your target specialty.

    Why switching later creates problems

    Switching from Innere Medizin to endocrinology after 3 years is a normal move. The problem starts when your Logbuch, the training record, contains only general ward work and no endocrinology rotation. You still have a solid Internal Medicine base, but you need further certified months in an endocrinology and diabetology unit.

    A typical large hospital separates its diabetes ward, endocrine outpatient clinic, pituitary conference, thyroid ultrasound list, and metabolic clinic. If your contract is in cardiology or gastroenterology, you may see diabetes during night duty but you do not automatically receive endocrinology training time. The chamber certifies supervised work in the approved area, not the specialty name you write on your CV.

    Is endocrinology a Zusatzweiterbildung in your state?

    Usually, doctors aiming to become endocrine specialists take the Internal Medicine and Endokrinologie und Diabetologie Facharzt route, not a short add-on course. A Facharzt is a fully trained specialist. The word Zusatzweiterbildung means an additional qualification after a specialist title, but the available additional qualifications and their requirements differ between state chambers.

    That difference matters before you accept a job. Berlin, Bavaria, North Rhine-Westphalia, and Hesse publish separate Weiterbildungsordnungen, their training regulations. The Federal Medical Association's Muster-Weiterbildungsordnung is a model, not the certificate that your chamber issues. Your own chamber decides your application when you submit the Logbuch and certificates at the end.

    Choose the department by its training licence

    Choose a first Internal Medicine job that keeps endocrinology open. A general Internal Medicine department with diabetes patients, an endocrine consultation service, and a named senior doctor with an endocrinology training licence is stronger than a ward that offers only a vague promise of later rotation.

    Ask one precise question during the interview: "How many months toward Internal Medicine and Endokrinologie und Diabetologie can this department certify under the current training licence?" Then ask to see the rotation schedule. You need a written answer from the chief's office or the doctor responsible for training, not a casual answer in a corridor.

    What normally transfers when you switch

    Certified common Internal Medicine months normally stay with you when you move hospitals or change from cardiology to endocrinology. Keep every Weiterbildungszeugnis, the detailed training certificate, signed by the authorised trainer. It should name your dates, weekly hours, ward, outpatient clinic, night duty, procedures, and any absences.

    Months outside the approved area create the dispute. Research time, an observership, emergency work without the required supervision, and service on an unrelated ward do not fill a missing endocrinology rotation. Part-time work can count, but the chamber converts it according to the weekly hours and its own rules.

    What to do before you switch

    • Download your Landesärztekammer's current Weiterbildungsordnung for Innere Medizin and Endokrinologie und Diabetologie, then save the PDF with the publication date.
    • Email your state medical chamber: "I have completed 3 years in Internal Medicine at [hospital]. Which months can count toward Facharzt Innere Medizin und Endokrinologie und Diabetologie in my case?"
    • Ask the endocrinology department for the name of the doctor holding the Weiterbildungsermächtigung and the number of certified months available.
    • Collect a detailed Weiterbildungszeugnis from your current chief before your last working day, while your ward duties and rotation dates are easy to verify.
    • Compare open endocrinology jobs with the certified training months, not only with the hospital's reputation or city name.

    The answer is simple: start with strong Internal Medicine training, then move into a chamber-approved endocrinology post before your missing months become expensive years. Your 3 years are the base. The training licence and the signed certificates decide whether they lead to the specialist title you want.
