German doctors need months before work: Papers, language, licence and visa must move in order.

Working as a doctor in Germany takes calendar months. Your papers, language exam, licence decision and visa must happen in order.

Do not expect your diploma to lead to a ward shift within weeks. Plan the work in monthly blocks. Keep each paper moving while you prepare the next step.

Why the licence takes months

Your full doctor licence is called an Approbation. The licensing office in your chosen state checks your diploma and internship certificate.

It also checks your work history, health certificate and police certificate. Each paper must match the office checklist.

The office compares your medical training with German medical training. Major differences lead to the medical knowledge exam, called the Kenntnisprüfung.

Your German language certificate is a separate paper. Most states also require the language exam for doctors, called the Fachsprachprüfung.

Use these monthly blocks

These blocks show a working order. They are not office deadlines. A missing stamp or exam place changes the calendar.

  1. Month 1: Choose 1 state. Read its doctor licence checklist. Make a folder for every required paper.
  2. Month 2: Order your diploma, transcript, internship certificate, good standing certificate and work letters. Send papers for certified German translations.
  3. Month 3: Send the complete licence application. Keep copies of every form, translation and postal receipt.
  4. Month 4: Prepare for the medical language exam. Practise patient history, discharge letters and doctor-to-doctor handovers.
  5. Month 5 onwards: Answer every licensing office letter quickly. Prepare for the knowledge exam if the office requests it.
A 5-step monthly order from licence papers to the first German hospital shift.

Where the work permit fits

A temporary doctor work permit is called a Berufserlaubnis. It can let you work under conditions during the full licence process.

This permit is not automatic. The licensing office decides whether to issue it. The permit can limit your state, hospital or medical tasks.

A hospital contract helps with your visa papers. A hospital cannot give you a doctor licence.

The licensing office decides the licence. The German mission or foreigners office decides the visa.

What delays the first shift

The common delay is an incomplete file. One work letter without dates can stop the office check.

A health certificate from the wrong doctor also causes delays. A translation without the original paper can do the same.

The second delay is waiting for the licence file before studying German. Start language exam preparation while you collect certificates and translations.

The third delay is applying without a hospital list. Start your job search when you can explain your licence stage clearly.

Put that licence stage in your German CV and cover letter. A chief needs to see where your application stands.

What to do this month

  • Choose 1 state and download its doctor licence checklist today.
  • Ask your university for a transcript with subjects, grades and training dates.
  • Ask each employer for a letter with your job title, exact dates and clinical tasks.
  • Book the language exam required by your chosen state.
  • Save every licensing office email in one folder.

The fastest file is a complete file. Build the papers first. Then move the licence, language and hospital steps together.

Stand: 2026-09-28. This is a guide, not legal advice. Every German state has its own rules, and rules change. Check with your state's [medical chamber (Landesärztekammer)](https://www.bundesaerztekammer.de/aerztekammern) before you act.
