Pick the right German state for your licence
Choose your German licence state by your job or home plan, its language rules, and the office handling your papers.

Apply for your full doctor licence in the German state where you can show a real job or home plan. Do not choose a state because an old Facebook post called it fast.
Each German state has its own licensing office. That office checks your diploma, certificate of good standing, language papers, and medical knowledge exam file.
Why the state matters
The basic federal law is the same across Germany. Your state office still decides which papers it needs and how it handles your file.
Berlin uses the State Office for Health and Social Affairs. North Rhine-Westphalia uses district governments, such as the Cologne district government.
This changes where you send certified copies. It also changes who writes to you when one translation, police certificate, or medical certificate is missing.
Choose a state you can explain
Most offices expect a link to their state. A job offer from a hospital, a written interview invitation, or a home address gives that link.
A hospital in Kiel gives you a clear reason to apply in Schleswig-Holstein. A planned move to Cologne gives you a clear reason to apply in North Rhine-Westphalia.
Sending the same file to several states creates extra letters and confusion. Pick one office first. Build one complete file for that office.

Compare the language rule first
Every state needs German language papers for doctors. The exact accepted certificate and language exam route differ between state offices.
Berlin sends doctors to the medical language exam through the Berlin Medical Association. Schleswig-Holstein lists its own language proof rules on the state health page.
Do not pay for a general German exam before reading your chosen office page. A certificate accepted for one application may not replace the doctor language exam elsewhere.
Think about your first job
Your licence state does not need to be Munich, Berlin, or Hamburg. A small hospital in Mecklenburg-Vorpommern or Saxony can give you a job interview sooner.
But you still need a place to live. You need an address registration, a bank account, and time for hospital interviews after arrival.
Choose a city where you can attend the language exam and interviews. A cheap room near your hospital is more useful than a distant office with fewer papers.
Do this before you apply
- List 3 states where you would genuinely live or work.
- Open each licensing office page and save its paper list.
- Compare the accepted German certificate and doctor language exam.
- Ask a hospital recruiter for a written interview or job offer.
- Send one complete file to the office you chose.
The best state is the one where your papers, language exam, and first hospital job fit together. That choice prevents months of sending the same certificate twice.
